Information Storage Authorization

You authorize Farmers to store any information you provide, including, but not limited to, names, addresses, birth dates, driver’s license numbers, social security numbers, vehicle identification numbers (VINs), Bank Account or Payment Card information.

You certify that you are the owner or authorized signer for any Bank Account or Payment Card you provide, and you authorize the financial institution where any such Bank Account or Payment Card is held to honor any withdrawals or charges you authorize.


Online Account Access

My Farmers Account Registration

My Farmers refers to services on farmers.com where you may register for a My Farmers Account which will allow you to, among other things, electronically view your policy related documents and/or billing-related notices and documents as well as update your information. You agree to be bound by these My Farmers Account Terms of Use, in addition to the other Terms of Use for www.farmers.com. If you do not agree with any of these Terms of Use, you may not create a My Farmers account.

Online services are available on Farmers.com as a part of your acceptance of these My Farmers Account Terms of Use. By agreeing to these My Farmers Account Terms of Use for online services and upon enrollment in My Farmers, you will have the option to receive email regarding your policy and/or billing related documents until: (1) you elect to discontinue this service, (2) you are no longer insured by one of the Farmers insuring entities, or (3) the online services are terminated by Farmers. Farmers reserves the right, at its sole discretion, to modify, add or remove any portion of these Terms of Use related to online services, in whole or in part. If you continue to use the online services thereafter, your continued use of the services is your acceptance of the changes and agreement to be bound by the Terms of Use as amended. If you do not agree to the changes, you agree to discontinue your use of the online services.

Farmers reserves the right, at its sole discretion, to change or discontinue, temporarily or permanently, the online services at any time without notice. You agree that Farmers will not be liable to you or any third party for any modification or discontinuance of the online services.

You may cancel your enrollment in the online services at any time by sending a request via our "Contact Us" form on www.bradshawinsuranceagency.com or choosing the personal preference to deactivate. When a specific customer (User ID) is deactivated, The Bradshaw Agency will remove associated payment methods, associated automatic payments, and any associated scheduled payments for the User ID that is deactivated; this includes resetting to paper billing. The payment methods will not be visible; however, they will remain available to The Bradshaw Agency to process refunds. Disclosures regarding storage of payment information for future refunds are included in any Payment Authorization available through the online billing service.

To ensure that The Bradshaw Agency is able to provide high quality services that are responsive to your needs, you agree that The Bradshaw Agency employees may have access to your online account and records as reasonably needed to investigate complaints and to conduct the business of Farmers in providing services for you.

You understand and agree there are certain emails that may be sent whether you enroll in the paperless process or not.

Go Paperless

The Bradshaw Agency is pleased to offer electronic delivery of notices and documents applicable to your selected automobile, property, personal umbrella, life, and annuity policies. Only those policies and accounts that you have selected will be enrolled for this service.

By electing to enroll in electronic delivery (Go Paperless) you agree to be bound by these Terms of Use in addition to the Terms of Use for www.farmers.com. If you do not agree with any of these Terms of Use, you may not enroll in Go Paperless.

By enrolling in Go Paperless and consenting to electronic delivery, you accept and agree to the following:

To receive all available notices and documents applicable to your selected automobile, property, personal umbrella, life and annuity policies, including but not limited to prospectuses, supplements to prospectuses, annual and semi-annual reports, annual statements, privacy statements, quarterly reports, proxy statements, special announcements, declaration pages, endorsements and policy contracts (collectively “documents”) via electronic delivery. There may be some documents that we cannot deliver electronically due to legal and/or technological constraints. These documents will be delivered to you via the United States Postal Service (USPS) to your postal address.

Documents will be made available in PDF or HTML format, so you must have a valid email account, access to an Internet browser and Adobe® Reader® software (this software is available for download free of charge at www.adobe.com). If you wish to print documents, you must also have access to a printer.

As documents become available, we will send you a notification by email which will contain the security-enabled Internet address (URL) where the documents can be viewed, downloaded and printed. It is your responsibility to log in to view your documents. We will only deliver documents to the policy owner.

That you will maintain a valid, current email address and immediately notify us of any change in your email address. Ensure that your email is active and capable of receiving new emails. To do this, ensure that your email account has sufficient space for new emails and that your email server and spam-blocking software do not block our emails. We are not responsible for problems arising from emails sent to an inactive or out-of-date email address, unless we are solely negligent for using an incorrect address. If an email is returned to us as undeliverable, we will notify you to update your email address through your agent and/or www.farmers.com. We will also resend the email. If it is returned once again as undeliverable we will presume that you have withdrawn your consent, you will be unenrolled in Go Paperless, and all available policy related documents will be sent to you via the United States Postal Service (USPS) to your postal address. Note that you will always have the ability to access www.farmers.com and view your documents on demand.

You may update your information, such as changing your email address, at any time by logging into farmers.com.

We may continue to send paper copies of certain documents, such as annual statements, if required by federal or state law. Electronic delivery is not currently available for all documents. When documents become available for electronic delivery, we will initiate the electronic delivery process without any further action required by you.

Although there is no charge associated with enrolling in Go Paperless, you may incur costs associated with electronic access to the documents, such as usage charges from Internet access providers and or telephone service providers. Farmers is not responsible for any damages to your computer hardware or software; injury to you as a result of power failures or power spikes, or telephone or internet interruptions or other expenses in relation to your use of electronic delivery.

With regard to your selected variable life and annuity policies, you may request a paper copy of any document at no charge by contacting Farmers New World Life Insurance Company (“Farmers Life”) Service Center at 1-877-376-8008 or by writing to P.O. Box 724208, Atlanta, Georgia 31139.

With regard to your selected traditional life and annuity policies, you may request a paper copy of any document at no charge by contacting Farmers Life at 1-206-232-8400.

With regard to your selected automobile insurance policy, property insurance policy, or personal umbrella policy, you may request a paper copy of any document at no charge by contacting your Farmers Agent.

Your consent to enrollment in Go Paperless will remain in effect for each selected policy until termination or cancellation of the policy, or until you revoke your consent. Electronic delivery will remain in effect following any policy modification or renewal.

You may revoke your consent at any time by going to www.farmers.com, logging into My Farmers, and selecting Paperless options under the Documents tab; then click on the View/Choose My Paperless Options tab to unselect the policy(ies) you want to remove from paperless policy documents. You may also revoke your consent by contacting us at 1-877-327-6392, or by contacting your Farmers Agent. Upon un-enrolling, all subsequent insurance documents will be mailed to your postal address via USPS. (Please allow up to 24 hours to process your un-enrollment.) Revocation of your consent shall not affect the legal enforceability of documents provided to you before the effective date of un-enrollment.

If you elect to revoke your consent and no longer choose to receive documents electronically, the Go Paperless discount on your policy (if any) will be removed.

If, after electing to withdraw your consent to receiving documents electronically, you decide you want to once again enroll in Go Paperless, you may do so by going to www.farmers.com. and completing the enrollment process.

Should we make any changes to hardware or software requirements for Electronic Delivery such that you will no longer be capable of accessing or retaining your documents electronically, we will inform you of the revised hardware and software requirements. You will be requested to review the revised Terms of Use and your continued enrollment will serve as your consent to continue participating in the Electronic Delivery option according to the new requirements.

We reserve the right to modify these Go Paperless Terms of Use at any time. Your continued participation in Go Paperless will constitute your acceptance of any revisions to these Terms of Use. Please check these Terms of Use on www.farmers.com regularly.

We are not required to deliver information electronically and may discontinue electronic delivery in whole or in part at any time.

IMPORTANT STATE SPECIFIC DISCLOSURES AND CONSENT LANGUAGE
Specific disclosures and consent language may be required by certain states. Please read the disclosures or consent language below applicable to the state for which your policy is issued.

GEORGIA:
For policyholders of the state of Georgia who have elected to receive all mailings and communications electronically, you hereby agree as follows:

I AGREE TO RECEIVE ALL MAILINGS AND COMMUNICATIONS ELECTRONICALLY. SUCH ELECTRONIC MAILING OR COMMUNICATIONS MAY EVEN INCLUDE CANCELLATION OR NONRENEWAL NOTICES.

TENNESSEE:
The policyholder who elects to allow for selected policy documents, available notices and communications to be sent to the electronic mail address provided by the policyholder should be aware that the election operates as consent by the policyholder for notices to be sent electronically, which may include notice of nonrenewal and cancellation. Therefore, the policyholder should be diligent in updating the electronic mail address provided to the insurer in the event that the address should change.

Paperless Billing

Farmers’ Paperless billing refers to our services on farmers.com where you may electronically view bills and billing-related notices and documents. You agree to be bound by these Paperless billing Terms of Use, in addition to the other Terms of Use for www.farmers.com. If you do not agree with any of these Terms of Use, you may not enroll in Paperless billing.

By enrolling in Paperless billing and consenting to electronic delivery, you accept and agree to the following:

To receive all available billing documents applicable to your selected billing account, including but not limited to statements, notices and correspondence (collectively, “billing documents”) via electronic delivery. There may be some documents that we cannot deliver electronically due to legal and/or technological constraints. These documents will be delivered to you via United States Postal Service (USPS) to your postal address.

As billing documents become available, we will send you a notification by email containing the security-enabled Internet address (URL) where the materials can be viewed, downloaded and printed. It is your responsibility to log in to view your bills and billing-related documents. We will only deliver bills and billing-related materials to the billing accountholder.

Billing documents will be made available in PDF or HTML format, so you must have a valid email account, access to an Internet browser and Adobe software (this software is available for download free of charge at www.adobe.com). If you wish to print billing documents, you must also have access to a printer.

You will maintain a valid, current email address and immediately notify us any of change in your email address. Ensure that your email is active and capable of receiving new emails. To do this, ensure that your email account has sufficient space for new emails and that your email server and spam-blocking software do not block our emails. We are not responsible for problems arising from emails sent to an inactive or out-of-date email address, unless we are solely negligent for using an incorrect address. Note that you will always have the ability to access www.farmers.com and view your billing documents on demand.

You may update your information, such as changing your email address, at any time by logging into farmers.com.

You acknowledge and agree that any named insured on any policy attached to your selected billing account may (A) Access and view all billing documents via My Farmers secure access at www.farmers.com or (B) Enroll or deactivate paperless billing options for the selected billing account. You may request a paper copy of a bill or billing-related document at no charge by contacting your Farmers Agent.

You may incur costs associated with electronic access to the billing documents, such as usage charges from Internet access providers and or telephone service providers. Farmers is not responsible for any damages to your computer hardware or software; injury to you as a result of power failures or power spikes, or telephone or internet interruptions or other expenses in relation to your use of electronic delivery.

There is no charge associated with enrolling in Paperless billing. However, if you elect to revoke your consent for Paperless billing, any associated service fee discount on your account (if any) will be removed.

Your consent to enrollment in Paperless billing will remain in effect for each selected billing account until termination or cancellation of the billing account, or until you revoke your consent. Electronic delivery will remain in effect following any policy modification or renewal.

You may revoke your consent to electronic delivery bill presentation at any time by going to www.farmers.com, logging into My Farmers and selecting Paperless options under the Documents tab; then click on the View/Choose My Paperless Options tab to unselect the billing account(s) you want to remove from Paperless billing. You may also revoke your consent by contacting us at 1-877-327-6392, or by contacting your Farmers Agent. Upon un-enrolling, all subsequent billing documents will be mailed to your postal address via USPS. (Please allow up to 24 hours to process your un-enrollment.) Revocation of your consent shall not affect the legal enforceability of billing documents provided to you before the effective date of un-enrollment.

If, after electing to withdraw your consent to electronic delivery, you decide you want to once again enroll in Paperless billing, you may do so by going to www.farmers.com. and completing the enrollment process.

Should we make any changes to hardware or software requirements for electronic delivery such that you will no longer be capable of accessing or retaining your documents electronically, we will inform you of the revised hardware and software requirements. You will be requested to review the revised Terms of Use and your continued enrollment will serve as your consent to continue participating in the Electronic Delivery option according to the new requirements. We reserve the right to modify these Terms and Conditions at any time. Your continued participation in Paperless billing will constitute your acceptance of any revisions to the Terms and Conditions. Please check the Terms and Conditions on www.farmers.com regularly.

We are not required to deliver information electronically and may discontinue electronic delivery in whole or in part at any time.

IMPORTANT STATE SPECIFIC DISCLOSURES AND CONSENT LANGUAGE
Specific disclosures and consent language may be required by certain states. Please read the disclosures or consent language below applicable to the state for which your policy is issued.

GEORGIA:
For policyholders of the state of Georgia who have elected to receive all mailings and communications electronically, you hereby agree as follows:

I AGREE TO RECEIVE ALL MAILINGS AND COMMUNICATIONS ELECTRONICALLY. SUCH ELECTRONIC MAILING OR COMMUNICATIONS MAY EVEN INCLUDE CANCELLATION OR NONRENEWAL NOTICES.

TENNESSEE:
The policyholder who elects to allow for selected policy documents, available notices and communications to be sent to the electronic mail address provided by the policyholder should be aware that the election operates as consent by the policyholder for notices to be sent electronically, which may include notice of nonrenewal and cancellation. Therefore, the policyholder should be diligent in updating the electronic mail address provided to the insurer in the event that the address should change.
